The mechanism was mostly stuck. The pen was being passed around for people to try to figure out how it should fill.
It turns out to be an elegant version of a syringe filling pen. Many syringe filling pens were cheaply made with a plastic rod to move the plunger, and a long blind cap to cover the rod.
This version has a metal rod in two sections, that connect via a small threaded portion. The bulk of the rod is covered by a matching segment of barrel, and the rod is attached to the piston rod.
It is functionally similar to the Improved Post Pen, but with an attached short blind cap.
